RIDDOR Reporting Form

Use this form to escalate incidents for review where RIDDOR may apply. It is not a substitute for the official HSE online report. For QA testing, no field is mandatory.

Any site accident book entry, local incident record, contractor paperwork, first-aid record or other Laserfast on-site documentation must still be completed in line with site procedures.

Very relevant for RIDDOR review. Complete where the affected person is alive and absence from normal work is known or reasonably anticipated. This section hides when fatality is selected.

Section 5 — Medical / emergency response

Select all that apply. Use “none known / unsure” where the injury is still being assessed.

Section 7 — Dangerous occurrence / high-risk event prompt

Select all that apply. This helps flag oxygen, nitrogen, pressure, electrical and fire-related events for management review.
